Section A: Personal profile

A1. Administrative
A1.1 Photo and Profile - Please include a passport-sized
photo pasted onto a one-page short profile as to who you are
- like you would introduce yourself to the staff at a new school
in a more personal informal context.

A1.2 Your detailed professional CV

A1.3 Formal qualifications - Insert certified copies of all formal
academic qualifications in the order that you obtained them
(i.e. starting with the oldest ending with the latest on top).

A1.4 Non-formal qualifications - Insert certified copies of all
non-formal academic qualifications in the order that you
obtained them (i.e. starting with the oldest ending with the
latest on top). Non-formal qualifications are work related
courses that you did, e.g. computer training, counselling etc.

A2. SWOT analyses – You will be doing several SWOT analyses
in the course of the programme. File them here in the order
that you did them. Every SWOT analysis will also include a
reflection on the process; these must be attached to the
SWOT analysis and filed with it.

A3. School Development Plan (SDP) – One of the outcomes of
the programme will include that you either develop or critically
engage with your school’s SDP in the final phases of your
studies. All relevant documentation linked to the process must
be filed here. E
